Did you know that by completing your New Jersey State income tax you may help save or enhance the lives of up to 50 people? On the NJ 2012 Personal Income Tax forms, taxpayers are asked if they would like to make a contribution to various funds. You can “Check Off for Life” by entering Code 03 on Line 62, “Other Designated Contribution.” This indicates that you are giving directly to the “Organ and Tissue Donor Awareness Education Fund.” The law provides that a contribution to the fund can be made whether there is an overpayment of taxes or a balance due on New Jersey Income Tax.
So, on your tax return consider “Check Off for Life” which provides an easy and convenient way for the state’s taxpayers to support organ and tissue donation education. Today there are over 4,500 New Jersey children and adults on the waiting list for life saving organ transplants, and each year that number increases. Each day 18 people die waiting! Education programs supported by contributions from “Check Off for Life” raise awareness of the urgent need for and benefits of organ and tissue donation, and help ensure that those on the waiting list receive their transplants.
